Your Rights Under the Title VI Civil Rights Act of 1964.
HOPE Community Services, Inc. offers its programs and services without regard to race, color, or national origin in accordance with Title VI of the Civil Rights Act. Any person who believes he or she has been aggrieved by an unlawful discriminatory practice under Title VI may file a complaint directly with HOPE Community Services, Inc.

Any person who believes that he or she, individually, as a member of any specific class, has been subject to discrimination prohibited by Title VI of the Civil Rights Act of 1964, as amended, may file a complaint with the Title VI Coordinator with the Office of Civil Rights. Title VI complaints must be filed using the ODOT’s Title VI Complaint Form and submitted electronically mailed to Oklahoma Department of Transportation Civil Rights Division: Jenny Chong, 200 NE 21st Street, Room 1-C5, Oklahoma City, OK 73105.
A complaint may also be filed directly with the Federal Transit Administration by filing a complaint with the Office of Civil Rights, Attention: Title VI Program Coordinator, East Building, 5th floor-TCR, 1200 New Jersey, S.E. Washington D.C. 20590.
